Good practice publications for you to steal and re-invent with pride!!

The Improvement Network - East Midlands - encourages the spread of good practice - 'share with humility'.  The following are examples of good practice that we are happy for you to 'steal with pride' on the understanding that you 'accredit with gratitude!'

 

United Lincolnshire Hospitals NHS Trust's Simple Discharges Toolkit

We implemented the simple discharges toolkit as a way of focusing our attention on preventing delayed discharges and improving patient flow. This required a whole system approach with as much information as possible going to the patients.  The implementation of the discharge co-ordinator was proving to be successful on a number of wards, as was the Jonah Discharge Planning Tool and we felt that the toolkit would help the role out of those and other work currently being done to support making best use of beds. It has been approved by our patient and public forum and is being used in a number of PCT's.  Please see the document attached below for more information.

Communities of Practice

This is an exciting concept I am learning lots about at present.  Groups of people who practice in the same field (multidisciplinary) who regularly get together (physically and/or virtually) to learn from each other in a non-hierarchical environment. Follow the link above to find out more.

 

Health Action Zones (HAZ)

Click above to find the HAZ evaluation sheets (which include learning points) from the HAZ projects that were funded in the year 2002-2003.  Many of the projects have now continued.  Each of the 20  individual projects has a pdf file with the evaluation leaflet on - there is excellent learning associated with them.

Protocol based care

A tool to support NHS staff in developing protocol-based care has been prepared and published by the Modernisation Agency and National Institute for Clinical Excellence (NICE).

Developed around NICE guidance and other recognised standards, the newinformation pack is designed to help staff turn evidence into practice byaddressing the key questions of what should be done, when, where, and by whom.  The pack does not represent formal NICE guidance to the NHS, but aims to spread good practice. Aimed at professionals working in community, primary and secondary care, it offers practical guidance on how to develop protocols. It links this with the wider agenda for service modernisation, and suggests other sources of advice, knowledge and support. The National Electronic Library of Protocols and Care Pathways 

In addition, the NeLH also supports the delivery of protocol - based care through its National electronic Library of Protocols and Care Pathways (NeLPCP). The NeLPCP has launched a new service - the Care Pathways Know-How Resource which provides a guide to relevant literature and on-line resources on good practice, and is available at  www.nelh.nhs.uk/carepathways.asp.  The NeLH welcomes the opportunity to add material from other NHS and associated organisations.
