Patient Tracking

Patient Tracking involves the selection of a randomised group of emergency patients and the detailed noting of their carers and their relatives experiences throughout their contact with the emergency care system.  This diagnostic exercise is in the process of becoming more widely used by the Department of Health Modernisation Agency leading to the need for emergency services professionals to familiarise themselves with this process.  This document offers a guide to the Patient Tracking exercise, providing both procedural guidance as well as useful explanations of the whole process.
