To register as a facilitator, you will need to submit your details to Kate Sharpe. Please click on the attached document (below). This will open a Word document into which you can enter your details directly. Some sections can be filled in by simply clicking in the "box" beside the appropriate answer. A tick will appear in that box. If you wish to change your selection, you click again to clear the box.
When you have completed the form, please save the changes as a new document on your hard drive. You can then send an email to Kate attaching this document containing your information. We will confirm the receipt of your details by email, and add them to the database within one week. If you have any problems, please contact Kate, either by email or by telephone on 0115 968 4478. Forms can also be printed off, completed and faxed back to 0115 9684400 marked for the attention of Kate Sharpe.
