Focus
NHS in Trent - Building on Success 2005 will reflect the changes and improvements in the provision of healthcare in Trent and show how these are being mainstreamed, embedded and sustained.
Objectives
Building on Success 2005 will further develop a knowledge collaborative, supported by The Improvement Network, by:
Providing the networking opportunities
Stimulating innovative ideas
Making opportunities to glean knowledge and experience from peers
Showcasing and sharing what works
Building joined up services for our patients which are underpinned by very good practice
Rewarding good practice and excellence
Awards
One Award will be presented at the Awards Evening (26th May 2005) for each of the following categories:

Long term conditions – living well

Improving patient experience – paying attention to the small things that matter

Access to services – responding to demand

Promoting healthy lifestyles – preventing ill-health

Effectiveness, efficiency and quality – optimising use of resources

Communications in the NHS – have you got your message across?

Working partnerships – from rhetoric to action
-
Supporting healthcare through technological innovation

Cross cutting themes
There will be 2 cross cutting themes to this year’s event:
There will be awards for each of these themes to be chosen from the 32 nominated finalists by our Patient and Diversity panels and presented at the awards ceremony and dinner.
Rules and Conditions

All pieces of work must involve an NHS partner (this could be your organisation - Trust, PCT, Shared Services, etc) but may be led by a representative of another stakeholder organisation.

Most of the changes to services must have been undertaken in 2003/4, and any 'projects' mentioned must still be in operation.

Entries must be received by email to jacqui.fowler@tsha.nhs.uk no later than 5.00pm on Tuesday 30th November 2004.

The main body of the abstract must be no more than 1000 words in length (as stated on the form).

As these submissions will be published both on a website and in the event brochure they must be checked for spelling and grammatical errors before submission.

To comply with best practice, please use plain English and avoid the use of abbreviations.

The award monies (£1000 per category) for eventual winners will be paid to you by automatic transfer to your organisation and must be used by you/your team to further develop the service or to undertake learning and sharing activities.

The winners will be expected to share their experiences and practice to help others learn from their work.

All winning entries will be supported in making submissions for National Awards (e.g. Health and Social Care Awards etc.).

The judges' decisions will be final.
