Working partnerships – from rhetoric to action!

This category is to showcase exemplary partnerships at all levels. The partnership could be across organisations, departments, professional groups etc. You will need to show that:

  • You have the infrastructures that support the partnership e.g. local strategic partnership, local implementation team for a national service framework, multi-professional/disciplinary group etc.

 

  • You have researched the evidence-base before initiating the partnership and service change

 

  • The partnership contributed to joined up services which reflect good practice and improved service user/carer experience

 

  • The results have evaluated well and can evidence the commitment to spread and sustainability
