Showcasing your service changes

NHS in Trent – Building on Success 2005 is focused on consolidation, sustainability, spread and the embedding of good practice in the delivery of healthcare services across Trent.  This is your opportunity to show how your work contributes to the delivery of national and local priorities and provides a high quality healthcare service for the diverse population of Trent.

We are now calling for abstracts illustrating service changes in each of the following categories:

  • Long term conditions - living well

  • Improving patient experience - paying attention to the small things that matter

  • Access to services - responding to demand

  • Promoting healthy lifestyles - preventing ill-health

  • Effectiveness, efficiency and quality - optimising use of resources

  • Communications in the NHS - have you got your message across?

  • Working partnerships - from rhetoric to action

  • Supporting healthcare through technological innovation

You can showcase your work at the 2-day learning and sharing event by filling out the attached abstract form.  Your abstract will be the basis of the poster presentation that you will be expected to provide for this event, to be displayed in the exhibition area.  It will also be published in the conference brochure and on The Improvement Network’s website.  A random selection of submissions will also be asked to present verbally to the conference though this will neither inform nor prejudice the Awards nomination process.

Please complete the form attached below and return it by email to jacqui.fowler@tsha.nhs.uk by 5pm on Tuesday 30th November 2004.  This is a strict deadline that will not be extended.
