Rehabilitation

On Wednesday 18th May 2011 the Regional Rehabilitation Programme presented:

If not now, when? - Planning and delivering patient outcomes from the time of diagnosis

Venue - Trent Vineyard, Nottingham

 

The aim of the day was to identify the responsibilities of health and social care personnel to plan and deliver patient outcomes, within a regional strategy for acute and community rehabilitation.

 

The event provided guidelines on how to:
  • Plan and monitor patient-centred outcomes in both acute and non-acute conditions
  • Ensure transfers between healthcare settings are appropriate and efficient
  • Ensure transfers between healthcare and social care settings are appropriate and efficient 
  • Adopt current best practice within the region
  • Facilitate  understanding of the service user perspective
  • Engage in continuous future improvement through a regional Rehabilitation Network
 
The audience was made up of clinicians in primary and secondary healthcare and social care; provider managers; commissioners; service user representatives
