On Wednesday 18th May 2011 the Regional Rehabilitation Programme presented:
If not now, when? - Planning and delivering patient outcomes from the time of diagnosis
Venue - Trent Vineyard, Nottingham
The aim of the day was to identify the responsibilities of health and social care personnel to plan and deliver patient outcomes, within a regional strategy for acute and community rehabilitation.
The event provided guidelines on how to:

Plan and monitor patient-centred outcomes in both acute and non-acute conditions

Ensure transfers between healthcare settings are appropriate and efficient

Ensure transfers between healthcare and social care settings are appropriate and efficient

Adopt current best practice within the region

Facilitate understanding of the service user perspective

Engage in continuous future improvement through a regional Rehabilitation Network
The audience was made up of clinicians in primary and secondary healthcare and social care; provider managers; commissioners; service user representatives
