The Integrated Health and Social Care team is a partnership between NHS Nottingham City and Nottingham City Council Adult Support and Health. We provide Clifton residents with a flexible, responsive and person-centred, multi-disciplinary team approach for anyone over the age of 18 with a long term condition.
We deliver quality outcomes for adults through services that are efficient, effective and closer to home that help prevent avoidable hospital admissions. The integrated service also operates within a hub and spoke model with access to a comprehensive range of specialist community services.
Nottingham has a strong history of partnership working, both formal (e.g. integrated provision for Intermediate Care Services) and informal partnerships have formed to improve service delivery. There is a wealth of experience and commitment from all levels of staff to deliver more integrated services.
The joint vision is to provide comprehensive, high quality, community-facing and personalised health and social care services for adults. The services are flexible, sensitive, responsive and specific to the individual needs of the local population. The service is now being rolled out across Nottingham City.
We adopt a proactive preventative approach for our defined populations, identifying vulnerable people at risk of hospital admission or admission to long-term care and providing personalised timely interventions, co-ordinated care and support. This is supported by a single point of access.
We have made the service more efficient and effective by adopting new processes and technologies including a triage system and new shared roles with a joint Safeguarding approach. Early indicatorshave demonstrated that by working differently, reducing duplication and overlap and streamlining processes, the average time required for a nurse visit has been reduced from 1.5 hours to 20 minutes, alongside reducing the waiting time to be seen by a social worker. Through using resources effectively and efficiently over a 3-5 year period significant economies of scale will be achieved.
