The finalists and winners in this year's East Midlands Health and Social Care Awards are listed below. The regional winners were announced at the Regional Awards Reception that took place on October 7th in Nottingham. The winner of each national category will represent the East Midlands in the National Awards. The shortlist of finalists in the National Awards will be announced soon and the actual National Awards ceremony will take place on 18th November in Liverpool.

Think Home First Project - Northamptonshire County Council with local NHS, voluntary and independent sector organisations - Regional Winner and National Finalist! Winner of this category in the National Finals! This project also went on to win one of two special awards on the night (for Putting Patients and the Public First). All six national winners were entered for the special awards – so they were the winners of winners for this criteria.
